Dear Faculty and Staff Colleagues:

The electronic form for faculty and staff to submit COVID-19 vaccination and exemption documentation is now available in the MySlice portal. This can be completed on a laptop or desktop computer with the best results using either Chrome or Firefox browsers. Mobile devices are not supported at this time.

As announced last week, 鶹ƵUniversity will require all faculty and staff to submit documented proof of vaccination status or information supporting their application for a medical or religious exemption. This process will occur electronically and the documentation provided by you will be retained in a secured and confidential electronic file that is separate from your official employment records.

We ask that you submit your documentation as soon as possible to allow sufficient time for it to be approved prior to the beginning of the spring semester (Jan. 18, 2022). Detailed in this message are the processes for submitting documentation for review and verification by the University.
