Dear Students and Families:

As shared last week, the Barnes Center at The Arch will host flu clinics throughout October. To provide students with convenient access to the flu vaccine, appointments will be available every weekday beginning Monday, Oct. 11, through Friday, Oct. 29. In addition, a clinic is scheduled for tomorrow, Friday, Oct. 8, at the Skybarn on South Campus from 9 a.m.-noon.

How to Get Your Flu Shot on Campus

• Log into the with your SU email and password. (You will be prompted to complete Microsoft’s Multi-factor Authentication in advance of accessing the Patient Portal if you have not done so previously.)
• Select “Appts” on the main menu.
• In the drop-down section:
  • Under “Clinic,” select “Health Clinic.”
  • Under “Reason,” select “Flu Shot Clinic Appointment.”
  • Under “Provider,” select the location you will receive your vaccine:
    • For Oct. 11-29, select “Barnes Center 309 FluShots.”
    • For Oct. 8, select “SkyBarn Flu Shots.”
  • Select “Submit.”
• Select your appointment day and time.

Upon signing up, you will receive a confirmation to your �鶹Ƶ��University email with appointment details and needed forms.

IMPORTANT: Please bring your pharmacy benefit card and health insurance card with you to your appointment. The flu vaccine will be billed to your insurance. Most health insurance companies cover the cost of a flu vaccine in full. Students who have a copay or medical charge applied will be billed accordingly. Please know ability to pay will not impact your access to services. If you are actively enrolled in the �鶹Ƶ��University Student Health Insurance Plan, you do not need to bring your insurance information.

If You Get Your Vaccine at Another Provider

While the University is offering flu clinics, students may choose to receive their flu shot through other sources, such as a primary care provider or at a pharmacy. If you get your flu shot elsewhere or you already received a flu shot after Aug. 1, 2021, you must provide proof of your vaccine in the Patient Portal.

• Log into the with your SU email and password. (You will be prompted to complete Microsoft’s Multi-factor Authentication in advance of accessing the Patient Portal if you have not done so previously.)
• Select “Upload” on the main menu.
• Follow the upload instructions.
• Select “Flu Vaccine Documentation” from the drop-down.
• Add your proof of vaccination.

If You Have a Medical or Religious Exemption

Students who already have a medical or religious exemption on file do not need to take any further action. If you need to waive the flu vaccine requirement for a medical or religious reason and do not yet have an exemption on file, please submit a Medical or Religious Waiver form through the Patient Portal.

• Log into the with your SU email and password. (You will be prompted to complete Microsoft’s Multi-factor Authentication in advance of accessing the Patient Portal if you have not done so previously.)
• Select “Forms” from the main menu.
• Under Student Forms, select the “Medical-Religious Waiver” form to print the waiver to sign.
• Once you have signed the waiver, select “Upload” on the home screen.
• Follow the upload instructions.
• In the drop-down section, select “Medical or Religious Waiver Form.”
• Select the completed waiver file to upload.
